{"id":1437,"date":"2026-03-05T10:00:00","date_gmt":"2026-03-05T10:00:00","guid":{"rendered":"https:\/\/staging.fonafis.bf\/focus-on-strategic-purchasing-of-healthcare-services\/"},"modified":"2026-03-05T10:00:00","modified_gmt":"2026-03-05T10:00:00","slug":"focus-on-strategic-purchasing-of-healthcare-services","status":"publish","type":"post","link":"https:\/\/www.fonafis.bf\/en\/focus-on-strategic-purchasing-of-healthcare-services\/","title":{"rendered":"Focus on strategic purchasing of healthcare services"},"content":{"rendered":"<p>Among the 27 FONAFIS 2026 sessions, <strong>session S22<\/strong> &#8211; scheduled for the morning of the third day (Friday, March 27) in the plenary room &#8211; will focus on a fundamental theme: <em>&#8220;Resource allocation and strategic purchasing: how to direct funding towards health priorities and pay providers based on results?&#8221;<\/em><\/p>\n<h2>What is strategic purchasing?<\/h2>\n<p>Strategic purchasing differs from passive purchasing &#8211; which simply reimburses costs &#8211; in that it links the payment of healthcare providers to their <strong>performance and the quality of the services delivered<\/strong>. It involves deciding which services to buy, from which providers and at what price, in order to maximize the health impact of each franc invested. <\/p>\n<p>The framework document to be presented at this session proposes a <strong>mixed payment<\/strong> system combining capitation, results-based financing (RBF) and fee-for-service according to service categories, with the CNAMU positioned as the main strategic purchaser within the RAMU framework.<\/p>\n<h2>Results-based financing in Burkina Faso<\/h2>\n<p>Burkina Faso has been experimenting with results-based financing (RBF) for several years. The session will provide a <strong>critical assessment of<\/strong> this experiment, examining the results obtained in terms of quantity and quality of care, the costs of implementation, and the difficulties encountered, such as payment delays, cumbersome verification and certain perverse effects. The articulation between the FBR and the newly launched RAMU will also be at the heart of the discussions.  <\/p>\n<h2>Towards a fairer allocation of resources<\/h2>\n<p>The session will also address the <strong>principles of equitable allocation of<\/strong> healthcare resources. At present, significant regional disparities persist in the distribution of resources. The framework document proposes the development of a <strong>weighted allocation formula<\/strong> (adjusted capitation), taking into account population, age structure, poverty level, rurality and accessibility conditions, particularly in areas affected by insecurity.  <\/p>\n<h2>Testimonials from the field<\/h2>\n<p>The session will include a <strong>panel of testimonials<\/strong> from players in the field: a CNAMU representative will describe the experience of the first months of contracting with providers within the RAMU framework; a public hospital manager will explain the challenges of contractualization and performance; a health district manager will share the realities of resource allocation at the operational level; and a private provider will present the conditions for a successful partnership with the public system.<\/p>\n<h2>A central issue for financing reform<\/h2>\n<p>Strategic purchasing is one of the pillars of the healthcare financing reform supported by FONAFIS 2026. As the forum&#8217;s framework document reminds us, the efficiency of the healthcare system depends not only on the volume of resources mobilized, but also on how they are allocated and how services are purchased.
